Clinical Evaluation for Tablet Characters and Palatability of Orally Disintegrating Tablets of Pitavastatin Calcium (LivaloⓇOD Tablet)

This study aimed to evaluate tablet characteristics and palatability of orally disintegrating tablets of pitavastatin calcium (LivaloⓇOD), which is a newly developed OD formulation of statin. Tablet hardness of Livalo OD 1 mg and 2 mg were 41.6±2.6 and 41.8±8.0 N, respectively. Clinical disintegration time, which determined in 11 healthy volunteers, were 17.9±5.4 sec for the 1 mg tablet and 22.1±6.2 sec for the 2 mg tablet. No damage to tablets was found after repeated packing of Livalo OD using automatic tablet packaging machine. When unit-dose packages of Livalo OD were stored for 28 days in 30℃ and 56%RH, the hardness of tablets decreased. However, food container and tee caddy with desiccant prevented the decrease of hardness of tablets. Visual analogue scale score for bitterness of Livalo OD 2 mg was under 30 mm, and was not change by the storage. In conclusion, this study showed that Livalo OD has a clinically sufficient tablet characteristics and palatability.

Drug Use Investigation Report on The Outcome of Kenketsu VenilonⓇ-I for Guillain-Barré Syndrome

献血ベニロンⓇ-Iがギラン・バレー症候群(GBS)に対する効能を取得後,10年間に収集された使用成績調査票を集計・解析し,安全性および有効性を評価した。
安全性に関する検討では,安全性解析対象症例1,184例のうち380例に696件の副作用が認められ,副作用発現症例率は32.1%(380/1,184)であった。投与毎(以下,エピソード)での副作用発現率は,初回投与エピソード30.5%(356/1,169),再投与エピソード24.7%(41/166)であり,再投与による副作用発現率の上昇は認められなかった。
主な副作用は,アラニン・アミノトランスフェラーゼ(ALT)増加90件,肝機能異常89件,アスパラギン酸アミノトランスフェラーゼ(AST)増加77件,白血球数減少35件であった。
副作用696件のうち,重篤な副作用は41例48件で,重篤な副作用発現症例率は3.5%(41/1,184),主なものは無菌性髄膜炎6件,肝機能異常6件,白血球数減少5件,好中球数減少5件であった。
有効性評価には,① Hughes運動機能尺度(Functional Grade;FG)1段階以上改善率(以下,改善率)と② FG 1段階改善日数(Kaplan-Meier 曲線による50%推定値,以下,改善日数)の2通りの指標を用い,エピソード別に評価した。
初回投与エピソードの改善率は69.4%(761/1,097),改善日数は12日,再投与エピソードの改善率は53.8%(84/156),改善日数は28日以上であり,再投与エピソードに比べて初回投与エピソードの方が,より良好な結果であった。初回投与エピソードでの年齢別の改善率は,小児84.9%(45/53),成人72.6%(571/786),高齢者55.9%(143/256),改善日数は小児7日,成人11日,高齢者21日であった。今回の調査結果による有効性は,臨床試験の初回投与エピソードにおける改善率:小児81.8%(9/11),成人60.9%(14/23),および改善日数:小児10日,成人14日と比べて,ほぼ同等であった。

Drug Use Investigation Report on The Outcome of Kenketsu VenilonⓇ-I for Guillain-Barré Syndrome

After receiving approval for our product Kenketsu VenilonⓇ-I for its use for Guillain-Barré syndrome, we've gathered information on its use for Guillain-Barré syndrome (GBS) for ten years, analyzed it, and evaluated safety and efficacy of the product.
Regarding safety, 380 out of 1,184 subjects whose data were analyzed for safety had had 696 cases of side effects; the rate of side effect occurrence was 32.1% (380/1,184). The rate of side effect occurrence per administration (hereafter referred to as episode) was 30.5% (356/1,169) for the first episodes and 24.7% (41/166) for the recurrent episodes. Readministration wasn't associated with side effect increase.
The frequent side effects in our investigation were increased alanine aminotransferase (ALT) (n=90), liver dysfunction (n=89), increased aspartate aminotransferase (AST) (n=77), and leukopenia (n=35).
Out of the 696 cases of side effects, 48 cases in 41 patients were serious; the rate of serious side effect occurrence was 3.5% (41/1,184). Frequent serious side effects, were aseptic meningitis (n=6), liver dysfunction (n=6), leukopenia (n=5), and neutropenia (n=5).
We used two measurements--1) the rate of improvement by more than one Hughes functional grade (hereafter referred to as improvement rate) and 2) the number of days needed to improve one grade of functional grade (50% estimates based on Kaplan-Meier Curve, hereafter referred to as number of days till improvement)-- and evaluated efficacy by episodes. The improvement rate for first episodes was 69.4% (761/1,097), and the number of days till improvement for the first episodes was 12.
The improvement rate for recurrent episodes was 53.8% (84/156), and the number of days till improvement for the recurrent episodes was more than 28. The outcome of the first episodes was better than the one of recurrent episodes. By age groups, the improvement rate for the first episodes was 84.9% (45/53) for children, 72.6% (571/786) for adults, and 55.9% (143/256) for elderlies, and the number of days till improvement was 7 for children, 11 for adults, and 21 for elderlies. The improvement rate during our clinical trial was 81.8% (9/11) for children and 60.9% (14/23) for adults for first episodes, and the number of days till improvement was 10 for children and 14 for adults. Therefore, efficacy of this Drug Use Investigation was almost equivalent to the one of the clinical trial.
