背景:本研究は,当院の2型糖尿病患者における2020年の日常診療下でのデータを,横断的に後ろ向き調査を実施し,患者プロファイルについて検討した。
方法:2020年1月~2020年12月までに当院を受診した成人2型糖尿病患者4,060例を対象とし,年齢別,HbA1c別,Body mass index(BMI)別の患者数,患者比率をそれぞれ男女別に解析し,糖尿病治療薬の処方率および処方数,処方比率を男女別,年齢別,HbA1c別およびBMI別に解析した。
結果:患者の平均年齢は67.6±12.7歳であり,65歳以上が約65%を占め,そのうちの約5割が75歳以上であった。HbA1cは平均7.1±1.0%であり,男性ではHbA1c高値(9%以上)の患者割合は30~35歳をピークに加齢とともに減少し,またHbA1c低値(6%未満)の患者割合は40歳未満の20%前後から40歳以上で10%前後へと減少した。HbA1c別の患者割合は男女で類似していたが,HbA1c高値の割合は全体的に男性の方が高い傾向にあった。BMIは平均24.4±3.9 kg/m2であり,男女ともBMI高値の患者割合は加齢とともに減少し,BMI低値では増加する推移を示した。糖尿病治療薬の処方率はα-グルコシダーゼ阻害薬(α-GI),メトホルミン,DPP-4阻害薬,インスリン,GLP-1受容体作動薬,グリニド薬,SGLT2阻害薬,チアゾリジン薬,スルホニル尿素(SU)薬の順に高かった。また,α-GIは年齢,HbA1c,BMIに関わらずほぼ一定の処方率であったが,高齢痩せ型の患者では主にインスリン,グリニド薬,チアゾリジン薬,SU薬およびDPP-4阻害薬が処方され,特にグリニド薬の処方率は10.3%と比較的高かった。
結論:2型糖尿病患者において,全体的には高齢化が進み,加齢とともにBMIは低下した。糖尿病の治療は個人差や多様性を加味して,個々の患者に最適の医療を考慮するが,特に高齢痩せ型の患者では血糖管理とともにサルコペニア,フレイルのリスク予防に留意した体重管理が重要と考えられた。

背景:SGLT2阻害薬は血糖改善効果以外にも肝機能改善効果が報告されているが,長期での検討や線維化への影響についての報告は数多くない。
方法:いわき内科クリニック(当院)通院中の2型糖尿病患者でトホグリフロジンが投与された105例を対象とし,後ろ向きに肝機能を含む各種臨床検査値を調査した。また,肝線維化の指標としてFIB-4 indexを算出した。
結果:最長104週の観察においてHbA1cや体重の低下とともに,AST,ALT,γ-GTPといった肝機能の有意な改善が認められた。FIB-4 indexは104週後において有意に低下し,線維化の改善が認められた。層別解析にて,特にFIB-4 indexの投与前値が1.3以上と線維化リスクが中等度以上の症例においてFIB-4 indexが有意に低下していた。
結語:トホグリフロジンは長期にわたる肝機能の改善に加え,肝線維化が改善する可能性が示唆された。

新型コロナウイルス感染症(以下,COVID-19)の流行下で日本人の成人てんかん患者が求めた情報を明らかにすることを目的として,アンケート調査を2020年7月1~ 3日及び10月15~20日に実施した。調査の対象は,楽天インサイトのスペシャルパネルにてんかん患者として登録されていたアンケートモニターで,2019年12月末までにてんかんと診断され,調査時点でてんかんの治療を受けている18歳以上の患者とした。調査項目は,患者背景に関する設問及びCOVID-19流行下でてんかん患者が求めた情報で構成された。患者が求めた情報に関しては,2020年1月9日~5月25日の間に,患者自身にとって各情報がどの程度必要であったかを「非常に必要・かなり必要・少し必要・あまり必要でない・不要」の5段階で評価した。各回の調査の回答者数は400名で,いずれも男性が約2/3を占めた。COVID-19流行下で患者が求めた情報の調査において,5段階評価で「非常に必要」又は「かなり必要」と回答した患者の割合(以下,必要度)が最も高かったカテゴリーは「日常生活」への影響について(初回調査及び第2回調査それぞれ49.8%及び46.0%)で,次いで高かったのは「COVID-19」について(初回調査及び第2回調査それぞれ48.0%及び45.8%)であった。さらに,患者背景因子別のサブグループ解析を実施した結果,てんかん発作が悪化し,かつCOVID-19の影響により検査・投薬・手術のいずれかに何らかの変更が生じたと回答した患者(n=9)では,「COVID-19とてんかんとの関連」及びCOVID-19流行下での「てんかん治療」に関する情報の必要度が高かった(各88.9%)。

Information Needed by the Japanese Patients with Epilepsy During the Coronavirus Disease 2019 Pandemic: Questionnaire Surveys Using the Internet

To clarify which type of information was needed by the Japanese adult patients with epilepsy during the coronavirus disease 2019 (COVID-19) pandemic, we conducted questionnaire surveys 1) from July 1 to 3 and 2) from October 15 to 20, 2020. Patients aged at least 18 years were eligible for each survey if they had been registered in the special panel of the Rakuten® Insight as a patient with epilepsy, if their illness was diagnosed as epilepsy by the end of December 2019, and if they had started receiving the treatment of epilepsy before the survey. Items of the questionnaires were composed of those 1) about the baseline characteristics and 2) about the type of information which the patients needed to know during the COVID-19 pandemic (from Jan 9 to May 25, 2020). As for the type of information, patients rated the degree of its demand using the 5-point scale (extremely needed, moderately needed, sometimes needed, less needed, and not needed). Four hundred patients participated in each survey and about two-thirds of the participants were men. In the analysis of the type of information, the highest proportion of patients (49.8% and 46.0% for the first and second survey, respectively) rated daily life as "extremely needed" or "moderately needed" in both surveys. COVID-19 was the second most needed type of information (48.0% and 45.8% for the first and second survey, respectively). The subgroup analysis revealed that patients who experienced the aggravation of seizure and any change in the examination, treatment, or operation owing to the pandemic (n=9) highly needed the information about the association of epilepsy with COVID-19 and the treatment of epilepsy during the pandemic (88.9% each).

大麻は古来より疾患の治療に用いられていたが,長い間その薬理作用は不明であった。しかし,内因性カンナビノイドとカンナビノイド受容体の発見により1990年代以降に徐々にそれが解明されてきた。
大麻草にはカンナビノイドと呼ばれる薬理作用を持つ成分が120種類以上含まれており,その中心となっているのがΔ-9テトラヒドロカンナビノール(以下THC)とカンナビジオール(以下CBD)である。THCは向精神作用から規制の対象となっているが,CBDは向精神作用を持たず,乱用の危険性もない。その安全性と臨床効果が近年注目され,急速に研究が進んでいる。CBD製品は海外では医薬品として難治性のてんかんに使用されているが,日本ではCBDに対する医師の認知度が低く,医療で使用される機会はほとんどない。
本論文では,医療用途の大麻の歴史,CBDの薬理学とその臨床応用について述べる。

Cannabis plants and resins have been used for therapeutic purposes since ancient times, but those pharmacological actions have long been unknown. However, the discovery of endocannabinoids and those receptors has been gradually elucidated since the 1990s.
The cannabis sativa containing more than 120 cannabinoids has pharmacological actions, the main of which are Δ-9 tetrahydrocannabinol (THC) and cannabidiol (CBD). THC is regulated for its psychotropic effects in almost all countries, but CBD does not have psychotropic effects and no risk of abuse in recent research. So, the safety and therapeutic potential of CBD have been attracting attention. The CBD products are used as medicines for refractory epilepsy in other countries, still there are a few chances for medical use in Japan because the awareness of CBD among physicians is very low.
This review will focus the history of cannabis for medical use, the pharmacology and therapeutic potential of CBD.

Objective: The objective of this study was to examine how the consecutive ingestion of Kikuimo (Jerusalem artichoke) containing inulin affected postprandial plasma glucose level.
Methods: A randomized, placebo-controlled, double-blind study was conducted. In this study we measured PBG after glucose tolerance twice at 0-week and 6-week, directed to healthy men and women aged from 20 to 59 y.o. with FBG of 125 mg/dL or less and/ or PBG of 199 mg/dL or less. Furthermore, adverse events were collected by means of a written questionnaire during the study.
Results: Total of 47 subjects took the first glucose tolerance test, to get 6-week ingestion started. 4 were withdrawn due to their own circumstances, and the remaining 43 completed the study. The 5 subjects whose blood lipid levels failed to meet the standard value were rejected from analysis. Thus, data obtained from 38 (active; 21, placebo: 17) was used for the analysis of efficacy. After 6 weeks, there were significant differences between active and placebo, in 60-min PBG and blood glucose AUC0-90min. No adverse events were observed.
Conclusion: We found out that the ingestion of inulin rich Kikuimo contributed to inhibiting rise of PBG. In addition, no safety-related matter occurred during the test period and the safety of the test foods were confirmed.
