2型糖尿病の治療剤であるインクレチン製剤のうち，今日DPP-4（dipeptidyl peptidase-4）阻害剤が多用されているが，使用経過中に類天疱瘡等の皮膚症状が報告されている。軽症の皮膚掻痒症は経験上多数認められるが，今回，DPP-4 阻害剤から経口GLP-1（glucagon-like peptide-1）製剤セマグルチドへ切り替えたことで，皮膚掻痒症が寛解した2 型糖尿病症例を経験したので報告したい。両製剤の作用機序の違いを考慮すると，皮膚症状等の副反応がより少ないことが期待できるセマグルチドは，有効な選択肢となる可能性がある。

Lots of type 2 diabetes patients are prescribed DPP-4inhibitor in Japan. We experience 2 diabetes cases that switch from DPP-4 inhibitor to oral glucagon-like peptide 1 (GLP-1) receptor agonist, Semaglutide, 3 mg showed improvement of dermal probrems itching. The DPP-4 inhibitor acts the elevations of intrinsic GLP-1, bradykinin, Neuropeptide Y, Substance P and so on substrates and has variety dermal problems including bullous pemphigoid and severe icthing. Considering the difference in the mechanism of action of both drugs, Semaglutide which is expected to have fewer side reactions, may be an effective option in type 2 diabetes.
