背景と目的:ナタリズマブ(NTZ)は世界で広く用いられている多発性硬化症(MS)の疾患修飾薬(DMD)の一つである。日本では2014年に承認され,実臨床下での有効性と安全性の検討を目的にNTZ投与患者全例を対象とした観察期間2 年間の製造販売後調査(全例調査)が現在進行中である。本研究では同調査の中間解析を行い,日本人のNTZ投与患者におけるMS年間再発率(ARR)改善効果と患者背景の関係を検討した。
方法:進行中のNTZ製造販売後調査3年目の中間集計データを用い,NTZ投与開始前と開始1年後のARRを患者背景別に比較検討した。
結果:有効性解析対象162名のうち,再発寛解型MS(RRMS)のNTZ新規投与患者(NTZnaïve RRMS患者)は110名であった。有効性解析対象全体,NTZ-naïve RRMS患者,さらにNTZ-naïve RRMSについて,NTZ投与1年前の再発回数,総合障害度(EDSS),過去に使用したDMD数,直近使用のDMDを指標にサブグループで検討した結果,いずれもNTZ投与によってARRは有意に改善した。
結論:NTZは,疾患活動性,EDSS,DMDの使用状況に関係なく日本人RRMS患者のARRを一様に低く抑制することが実臨床下で示された。さらに,DMD未使用患者であっても疾患活動性の高い場合にはNTZが早期に選択されており,添付文書や診療ガイドラインに記載通りの適正使用が確認されたとともに,その有効性も確認された。

Background and Aim: Natalizumab (NTZ) is a disease-modifying drug (DMD) that has been used worldwide for the treatment of multiple sclerosis (MS). In Japan, NTZ was approved in 2014, and all-case post-marketing surveillance with a 2-year observation period enrolling all patients who took NTZ is currently ongoing to investigate the effectiveness and safety of NTZ in the clinical setting. By performing the present interim analysis of the surveillance results, we aimed to investigate the association between improvement in MS annual relapse rate (ARR) and background factors in Japanese patients in patients with natalizumab.
Methods: Using the 3-year interim data from the post-marketing surveillance, ARRs before NTZ treatment and after 1 year were analyzed according to patient background factors.
Results: Of 162 patients included in the effectiveness analysis, 110 were NTZ-naïve patients with relapsing-remitting multiple sclerosis (NTZ-naïve RRMS patients). After NTZ treatment, ARRs were significantly improved in all patient groups i.e. the effectiveness analysis population, NTZ-naïve RRMS patients and subgroups of NTZ-naïve RRMS patients according to the number of relapses at 1 year before NTZ treatment, expanded disability status scale (EDSS) score, number of prior DM use and the most recent DMD use.
Conclusions: NTZ reduced ARR to a low level in Japanese RRMS patients in the clinical setting consistently regardless of disease activity, EDSS score or prior DMD use. It was also shown that NTZ was being used at the early stage even in DMD-naïve patients in the case of high disease activity, in accordance with the package insert and clinical guidelines, and effectiveness of NTZ was confirmed.

Epidemiological Survey of Erectile Dysfunction (ED) Patients in ED & AGA Clinic in Tokyo: Patient Background and Risk Factors Evaluation from 1567 ED Cases

目的:ED専門クリニック(自由診療)初診患者における疫学的調査(簡易)を実施し,リスクファクター等の実態調査をすること。
方法:本調査は,医療法人社団イースト会イースト駅前クリニック新橋院における初診時問診票と電子カルテをRetrospectiveに検索し,1576症例から患者年齢,基礎疾患,治療歴(併用薬等含む),喫煙・飲酒等を調査した。
結果:調査期間は2018年1月4日から4月15日までの約4カ月間。対象患者数は当院初診患者でデータ欠測のない1576症例とした。初診時の年齢は20歳から83歳。40歳代後半がピークで,平均年齢47.8±34.2歳であった。基礎疾患の有無では,ありが30%,なしが70%であった。疾患領域別では,高血圧症が35%と最も多く,次いで糖尿病が15%,高脂血症4%,高尿酸血症3%,その他43%であった。喫煙(電子タバコ含む)の有無では,喫煙者33%,非喫煙者65%,不明2%であった。飲酒に関しては,休肝日ありを含めた飲酒者が74%,非飲酒者が25%,不明1%であった。
結論:本調査からEDのリスクとして加齢以外に,高血圧症,糖尿病,高脂血症,高尿酸血症などいわゆるメタボリック症候群の合併が多かった。また基礎疾患に対する治療薬剤(降圧剤,抗糖尿病剤,精神科系薬剤など)による,いわゆる「薬剤性ED」が加齢リスクに重複していることも示唆された。喫煙に関しては,EDのリスクファクターであることはすでに報告されており,喫煙者に対しては単にPDE5阻害剤の処方にとどまらず,禁煙教育を同時に実施することも重要であろう。また若年層の受診も近年増加傾向にあり,本調査でも20代と30代が合わせて全体の27.6%を占めており,心因性なども含めたEDのリスクファクターの多様化が示唆された。今後の課題として,PDE5阻害剤無効例(ノン・レスポンダー)に対する性腺機能検査(テストステロン等)さらに非薬物治療(低出力衝撃波療法)と併用療法の検討も示唆された。

Epidemiological Survey of Erectile Dysfunction (ED) Patients in ED & AGA Clinic in Tokyo: Patient Background and Risk Factors Evaluation from 1567 ED Cases

Objective: To conduct an epidemiological survey in the ED clinic (self-funded medical treatment) on first-visit patients and investigate their actual condition, including its risk factors.
Methods: This survey was conducted retrospectively using a first-visit questionnaire form and e-medical chart (East Clinic Patient Data Base) at the East Ekimae Clinic, Shinbashi, Minato-ku, Tokyo. We investigated age, underlying disease, treatment history (including concomitant drugs), and smoking/alcohol habit.
Results: The investigation period was about 4 months, from January 4, 2018 to April 15, 2018. The number of subjects was 1576 cases in our first-visit patient cohort. The age range at the time of the first visit was 20 to 83 years. The peak age was in the late 40s, with an average age of 47.8± 34.2 years. Underlying disease was present in 30% of cases, with hypertension being the most frequent accounting for 35% of underlying diseases, then diabetes 15%, hyperlipidemia 4%, hyperuricemia 3% , and others 43%. Furthermore, 33% of patients were smokers, including electronic cigarettes, 65% non-smokers, and 2% unknown. Regarding drinking alcohol habit, 74% of patients were drinkers, including non-drinking days, 25% nondrinkers, and 1% unknown.
Conclusions: This survey found that there were a substantial number of patients who suffered from "metabolic syndromes" such as hypertension, diabetes, hyperlipidemia, and hyperuricemia, aside from aging representing a risk of ED. It was also suggested that the so called "drug induced ED", due to therapeutic agents against underlying diseases (antihypertensive agents, antidiabetic agents, and psychiatric agents), overlaps the aging risk. Regarding smoking, it has already been reported that it is a risk factor of ED, and for smokers it is important not only to prescribe PDE 5 inhibitors, but also to implement smoking cessation education at the same time. In addition, the ratio of young people visiting the ED clinic also increased in recent years; in this survey, patients aged in their 20s and 30s together accounted for 27.6% of all the ED subjects indicating diversification of the ED risk factors, including psychogenicity. It was also suggested that a further examination of gonadal function test, such as Testosterone test., may be required for non-responders to PDE5 inhibitors, and a non-drug therapy, such as Low Intensity Shockwave therapy with/without PDE5 inhibitors may be necessary.

ジェネリック医薬品のミノドロン酸錠1 mg「サワイ」およびミノドロン酸錠50 mg「サワイ」と,先発医薬品であるボノテオⓇ錠1 mgおよびボノテオⓇ錠50 mgとの生物学的同等性を検証するため,日本人健康閉経後女性を対象に2剤2期のクロスオーバー試験を行った。
最終採血時点までの血漿中ミノドロン酸濃度より,両製剤の血漿中濃度-時間曲線下面積(AUCt)と最高血漿中濃度(Cmax)を算出し,バイオアベイラビリティを比較した。その結果,いずれの試験においても両製剤のAUCtとCmaxの対数値の平均値の差の90%信頼区間は「後発医薬品の生物学的同等性試験ガイドライン」の基準であるlog(0.80)~log(1.25)の範囲内であった。なお,本治験において認められた有害事象は,いずれも回復が確認されており,被験者の安全性に問題はなかった。
 したがって,ミノドロン酸錠1 mg「サワイ」およびミノドロン酸錠50 mg「サワイ」は,ボノテオⓇ錠1 mgおよびボノテオⓇ錠50 mgと治療学的に同等であると考えられた。

目的:シャンピニオンエキス,渋柿エキスを含有する「メンズデオ8400」の体臭改善効果を確認するため,非摂取を対照とした無作為化並行群間比較試験を行った。
方法:20歳以上50歳以下の健康な成人男性にメンズデオ8400を1日2カプセル,8週間継続して摂取させ,被験者の体臭(頭皮,うなじ,脇の下,足の裏)と口臭,前日に着用した肌着の臭気を測定,評価した。その他,体臭,口臭,便臭に関してアンケートによる被験者の自己評価と家族の評価を行った。
結果:40人を年齢に偏りがないように試験品を摂取するグループと非摂取のグループに20人ずつ割付け,試験を開始した。38人(摂取群18人,非摂取群20人)が試験を完遂し,解析対象は38人だった。臭気測定の結果,摂取群の頭皮,うなじ,脇の下,足の裏,口臭,肌着のすべての項目で,4週後と8週後のいずれも非摂取群と比べて有意に改善したか,悪化が抑えられた。被験者の自覚や家族の評価においても,試験品を8週間摂取すると体臭,口臭,便臭が改善したと実感したことが分かった。8週の試験期間中に有害事象は発生せず,試験品の安全性が確認された。

目的:美容液「セルアイLセラム」による乾燥小ジワ改善とたるみ改善効果の検証を目的とした。
方法:目尻のシワグレードが左右対称同程度で主として1-3のシワを有する30歳以上59歳以下の健康な女性を対象として,同一人の左右対称部位の片側(半顔)に試験品塗布,もう片側を無塗布とするハーフフェイス法(遮蔽試験)を行なった。試験品を毎日朝晩の2回,4週間継続して塗布させ,使用前と4週使用後の状態を評価した。アウトカムは,目尻のシワ,たるみ,肌弾力,フェイスライン角度,顔幅,主観評価(肌の潤い,肌の柔らかさ,肌のつや,肌のなめらかさ,肌荒れ,肌のキメ,化粧のり,肌のハリ・弾力,しわ,肌のたるみ,肌の毛穴,洗顔後のつっぱり感,肌の明るさ,総合的な肌の満足感)とし,試験品の安全性についても評価した。
結果:14人全員が試験を完遂し,14人を解析対象とした。試験品塗布側と無塗布側それぞれの使用前と4週使用後の変化量を群間比較したところ,塗布側は目尻のシワグレード,たるみ,肌弾力,フェイスライン角度,主観評価(14項目中13項目)において有意な改善がみられた。また,顔幅は,使用前から4週使用後に有意に減少(改善)した。
結論:美容液「セルアイLセラム」を4週間継続使用すると,目尻のシワ,たるみ,肌弾力,フェイスライン角度,顔幅が改善することが分かった。また試験品の安全性についても問題がないことが確認された。
